p-ANCA関連性血管炎に合併した視神経周囲炎の1例

Optic perineuritis in a case of vasculitis associated with perinuclear anti-neutrophil cytoplasmic antibodies（p-ANCA）

要約　73歳女性が5か月前からの緩慢な視力低下で受診した。視力は両眼とも手動弁で，右眼に視神経萎縮，左眼に軽度の視神経乳頭浮腫があった。画像診断で視神経鞘を含む視神経周囲組織に腫脹があった。血液検査で，抗好中球細胞質抗体（p-ANCA）が陽性であり，ANCA関連血管炎に合併した視神経周囲炎と診断した。副腎皮質ステロイドのパルス療法と免疫抑制剤による治療を行い，左眼視力はその翌日に0.2に改善した。以後，視力に動揺があったが，9か月後に左眼視力は0.7に回復した。しかし求心性視野狭窄が残った。右眼視力には変化がなかった。本症例での再発性視神経障害の発症と，炎症の再燃・寛解にp-ANCAが関与している可能性がある。

Abstract.　A 73-year-old woman presented with deteriorating vision since 5 months before. Her visual acuity was hand motion in either eye. Funduscopy showed optic atrophy in the right eye and disc edema in the left. Diagnostic imaging showed swelling in and around the optic nerve sheath. She was positive for perinuclear anti-neutrophil cytoplasmic antibodies（p-ANCA）in the blood,leading to the diagnosis of bilateral optic perineuritis associated with p-ANCA. She was treated with pulse corticosteroid and immunosuppressive agent. Her left visual acuity improved to 0.2 on day 2 of treatment. Nine months after her initial visit,the left visual acuity was 0.7 with constricted visual field. Visual impairment in her right eye remained unchanged. This case illustrates that p-ANCA may be involved in the pathogenesis and clinical course of optic perineuritis.
