要約　白内障手術を45例90眼に行い，各症例とも1眼にアクリル眼内レンズ（IOL），他眼にシリコーンIOLを挿入した。年齢は39歳から90歳，平均70.1歳であった。アクリルIOLは3.5mm強角膜切開から鑷子で挿入し，シリコーンIOLは3.0mm強角膜切開からインジェクタで挿入した。術翌日の視力は，シリコーンIOL挿入眼で有意に改善した。惹起乱視は，術後1か月以外ではアクリルIOL挿入眼のほうが有意に大きかった。これらの所見は70歳以上の患者で顕著であった。眼圧，予想屈折値との差，後発白内障は両群間に差がなかった。高齢者では切開創が小さく，ストレスが少ない術式を選択すべきである。

Abstract.　We performed cataract surgery on 90 eyes of 45 patients. Each patient received acrylic intraocular lens（IOL）in one eye and silicone IOL in the other. Their ages ranged from 39 to 90 years,average 70.1 years. Acrylic IOL was inserted through a 3.5 mm sclerocorneal incision with forceps. Silicone IOL was inserted through a 3.0 mm incision with an injector. Visual acuity on day 2 of surgery was significantly better in eyes receiving silicone IOLs. Induced astigmatism was greater in eyes receiving acrylic IOLs except one month after surgery. There were no differences between the two groups regarding intraocular pressure,refractive error from predilected value,or aftercataract.We advocate smaller incision and procedure inducing less stress for cataract surgery in elder patients.
