 We performed secondary intraocular lens(IOL)implantation in 50 eyes that had received vitrectomy and phacoemulsification-aspiration. Fourteen eyes received acrylic IOL and 36 eyes received silicone IOL. The initial diseases comprised proliferative diabetic retinopathy 20 eyes,retinal detachment 19 eyes,ruptured retinal arterial macroaneurysm 6 eyes,branch retinal vein occlusion 4 eyes,and idiopathic vitreous hemorrhage one eye. After 3months or later,the visual acuity improved by 2 lines or more in 16 eyes(32%),remained unchanged in 32 eyes(75%),and deteriorated in 2 eyes(4%). These two eyes comprised one eye each of progression of posterior capsular opacity and another episode of rupture of retinal macroaneurysm. Severe complications were consistently absent. Posterior iris synechia was initially present in 20 eyes and persisted in 3 eyes only after surgery. These three eyes received acrylic intraocular IOL. This incidence was significant when compared with eyes receiving silicone IOL(p<0.01). These findings show that secondary IOL implantation was safe and effective in the present series.
