短期シリコーンオイルタンポナーデの術後合併症

Short-term complications of silicone oil tamponade

　シリコーンオイルタンポナーデを行い120日以内に抜去した42眼での合併症を検索した。内訳は黄斑円孔23眼，裂孔原性網膜剝離12眼，糖尿病網膜症5眼，穿孔性眼外傷2眼である。術後合併症は，一過性眼圧上昇20眼，術後虹彩後癒着9眼，角膜内皮細胞密度減少7眼，後発白内障6眼，シリコーンオイル下膜増殖5眼，シリコーンオイル油滴の眼内レンズ付着5眼，視野異常3眼であった。後発白内障1眼を除き，視力が術前よりも悪化したものはなかった。さらに，術後視力を大きく障害する重篤な合併症もなかった。前房隔壁がない症例では角膜内皮障害が高率に生じた。眼内レンズまたは後囊による前房隔壁を温存することの重要性が示された。

　We reviewed 42 eyes that received silicone oil tamponade for 120 days or less. The series comprised macular hole 23 eyes,rhegmatogenous retinal detachment 12 eyes,diabetic retinopathy 5 eyes,and perforating injury 2 eyes. Following complications were observed：temporary ocular hypertension 20 eyes,posterior synechia 9 eyes,loss of corneal epithelial cells 7 eyes,aftercataract 6 eyes,membraneous proliferation posterior to silicone oil 5 eyes,adhesion of silicone oil to intraocular lens 5 eyes,and abnormal visual field 3 eyes. Except one eye of aftercataract,no eye developed visual loss below the pretreatment level. No case developed vision-threatening complication. Damage to corneal endothelium was frequent in eyes with insufficient partition between the anterior and posterior chamber,including intraocular lens or posterior capsule.
