高齢者ドナーを用いた全層角膜移植の検討

Evaluation of penetrating keratoplasty using corneas donated from octo-and nonagenarians

　80歳以上のドナー角膜を用いた全層角膜移植術の有用性について検討した。過去2年間にアイバンクから提供されたドナー角膜96眼の角膜内皮細胞をスペキュラマイクロスコープで観察した。ドナー角膜は80歳以上33眼，80歳未満63眼であった。別の検討として，1986～1998年の12年間に初回手術として全層角膜移植を施行した80眼の術後5年目での透明治癒率，術後視力を検討した。ドナー角膜の内訳は80歳以上22眼，80歳未満58眼であった。ドナー角膜の内皮細胞密度は，80歳以上では2,512個/mm²，80歳未満では2,720個/mm²であり，80歳以上のドナーで有意に少なかった（p＝0.04）。一方，全層角膜移植を行った80眼での5年後の透明治癒率と術後視力は，いずれも80歳以上と80歳未満との間で有意差が認められなかった。以上のことから，80歳以上の高齢者ドナー角膜を用いた全層角膜移植術は臨床的に有用である。

　Purpose：To evaluate the validity of penetrating keratoplasty（PKP）using corneas donated from persons aged 80 years or over. Cases and Methods：We examined endothelial cell morphology of 96 corneas obtained from eyebanks for two years with a specular microsope before PKP. Thirty-three corneas were donated from persons aged 80 years or over and 63 corneas from those below 80 years of age. In another series,we evaluated the outcome of penetrating keratoplasty in 80 patients during 12 years from 1986 to 1998. Twenty-two corneas were donated from persons aged 80 years or over and 58 corneas from those below 80 years of age. Results：Density of corneal endothelial cells averaged 2,512 cells/mm²for donors aged 80 years or over and 2,720/mm²for those aged below 80 years. The difference was significant（p=0.04）. There were no differences in the rate of graft transparency or averaged visual acuity 5 years after surgery between the two groups. Conclusion：Penetrating keratoplasty using corneas donated from octo-or nonagenarians is clinically useful.
