　緑内障，白内障合併例に対し，眼内レンズ挿入・線維柱帯切除同時手術を行った71眼について，Cox比例ハザードモデルによって術後眼圧コントロールに対する影響因子をretrospectiveに解析した。解析因子としては，患者年齢，性別，術前眼圧，線維柱帯切除時の線維芽細胞増殖阻害剤，白内障手術時の切開部位，緑内障病型，虹彩処置の有無，術後フィブリン析出の有無，白内障術式を選択した。解析結果として，白内障術式の中で水晶体嚢外摘出術による同時手術では，ハザード比2.6,p≦0.0042で有意に眼圧コントロールが悪化しやすくなったので，水晶体嚢外摘出術による同時手術は避けるほうが望ましいと考えられた。

We reviewed 71 eyes with glaucoma and senile cataract treated by simultaneous trabeculectomy and cataract surgery with intraocular lens implantation. We tried to identify prognostic factors relative to postoperative control of intraocular pressure, IOP. Cox proportional hazards model was used in the analysis. Following factors were evaluated: patient's age, sex, preoperative IOP, type of glaucoma, method and site of incision of cataract surgery, manipulation of the iris, postoperative fibrinous reaction, and intraoperative use of antiproliferative agent with mitomycin C or fluorouracil. Extracapsular cataract extraction signifi-cantly contributed to failure of postoperative IOP control with hazard ratio of 2.6, p≦0.0042.
