We evaluated the effect of topical 2% cyclosporin A (CsA) in preventing allograft rejection after penetrating keratoplasty in 42 eyes of 42 patients. Another series of 50 eyes of 50 patients without CsA served as control. Both groups were operated with the same surgical procedure and showed matching risk features as bullous keratopathy, neovascularization or regrafting. The rejection-free survival rate was calculated by Kaplan-Meier's life table analysis. The survival rate at 20 months was 87.3% for CsA group and 68.6% for control. The difference was significant. Topical CsA was thus effective in preventing allograft rejection after penetrating keratoplasty.
