全層角膜移植術後に外傷性創離開を起こす患者背景因子

Background factors of traumatic wound dehiscence following penetrating keratoplasty

　過去8年間に全層角膜移植術後の外傷性創離開が7例7眼に起こった。男性4例，女性3例で，年齢は23～79歳（平均51歳）であった。原疾患は円錐角膜2眼，角膜白斑2眼，水疱性角膜炎2眼，球状角膜1眼であった。4眼では受傷前の矯正視力が両眼とも0.1以下であった。創離開への処置後の最終視力は，4眼で受傷前よりも低下した。全層角膜移植術後に外傷性創離開が起こる患者側の背景因子として，両眼の視力不良，比較的若年で活動が活発である，角膜全体が菲薄な球状角膜などがあった。

　We reviewed 7 eyes of 7 patients that developed traumatic dehiscence of the corneal wound after previous penetrating keratoplasty. The series comprised 4males and 3 females. Their ages ranged from 23 to 79 years,average 51 years. The original diseases included keratoconus 2 eyes,corneal leucoma 2 eyes,bullous keratopathy 2 eyes,and keratoglobus one eye. The corrected best visual acuity was 0.1 or less in either eye before trauma. The final visual acuity was less than that before wound dehiscence in 4 eyes in spite of treatment. Following background factors were identified for wound dehiscence after penetrating keratoplasty：poor visual acuity in both eyes,being young and active,and keratoglobus with thin cornea.
