 Elevated vitreous pressure is one of risk factors for intraoperative complication during simultaneous penetrating keratoplasty,cataract surgery and intraocular lens implantation,or triple corneal procedure. We evaluated the predictive value of body mass index by reviewing 36 cases of this procedure during the past 13 years. In the second half of this period,perilimbal partial scleral incision was performed prior to surgery to facilitate eventual core vitrectomy in cases of body mass index exceeding 26.0. Presurgical eyeball compression for 30minutes instead of conventional 10minutes was effective in reducing intraoperative complications. There was significantly high incidence of complications and eventual vitrectomy in cases with high body mass index. The findings suggest that safety of triple corneal procedure can be enhanced by sufficient reduction in intraocular pressure and preparations for vitrectomy in cases with high body mass index.
