超音波乳化吸引術による白内障緑内障同時手術後の長期眼圧経過の生命表による検討

Simultaneous surgery for cataract and glaucoma with phacoemulsification. Postoperative course of intraocular pressure by life table analysis

　緑内障・白内障合併例に対し水晶体超音波乳化吸引術＋後房レンズ挿入術＋線維柱帯切除術による同時手術を行った40例47眼の眼圧経過について，生命表法を用いてretrospectiveに検討した。術後視力は0.5以上が57％，0.8以上が32％であった。術後合併症はフィブリン反応が最多で21％であった。術後眼圧は18か月で有投薬眼圧コントロール確率が89.0±4.9％，同無投薬眼圧コントロール確率が59.0±9.3％であった。また，生命表法による濾過胞の18か月での生存率は75.0±8.9％であった。これらの結果は筆者らが以前に報告した水晶体嚢外摘出術＋後房レンズ挿入術＋線維柱帯切除術の術後経過より良かった。

We performed phacoemulsification with posterior chamber lens implantation and trabeculectomy in 47 eyes with glaucoma and cataract. The series included primary open angle glaucoma 34 eyes, primary angle closure glaucoma 8 and secondary glaucoma 5. Postoperative visual acuity was 0.5 or better in 27 eyes, 57%, and 0.8 or better in 15 eyes, 32%. Fibrinous reaction developed in 10 eyes, 21%, as the most frequent early complication. We analysed of postoperative control of intraocular pressure (IOP) using the life-table method of Kaplan-Meier. The probability of successful IOP control at 18 months with medication was 89.0±4.9% and that without medication was 59.0±9.3%. The probability of functional filtering bleb at 18 months was 75.0±8.9%.
