肥厚性硬膜炎により眼球運動障害をきたした1例

A case of pachymeningitis with impaired ocular motility

目的：眼球運動障害が併発した肥厚性硬膜炎症例の報告。症例と経過：77歳男性が両眼性複視で受診した。8か月前から微熱と右眼充血があり，近医で強膜炎と診断され，副腎皮質ステロイド薬の点眼でいったん軽快していた。軽度の右眼球運動障害があった。1か月後に熱発し，右眼視力が低下し，強膜炎が再発し，眼瞼下垂と眼球運動障害が悪化した。造影磁気共鳴画像検査（MRI）で右の脳底部に髄膜肥厚があり，肥厚性硬膜炎と診断した。P-ANCA抗体が陽性であった。副腎皮質ステロイドのパルス療法後に内服漸減を行い，2か月目から免疫抑制薬を内服することで諸症状が改善した。結論：本症例の発熱と眼球運動障害は肥厚性硬膜炎に続発したと推定される。肥厚性硬膜炎の診断には造影磁気共鳴画像検査が有用であった。

Purpose：To report a case of hypertrophic pachymeningitis with impaired ocular motility. Case and Findings：A 77-year-old man presented with bilateral diplopia. He had had slight fever and hyperemia of his right eye since 8months before. He showed disturbed ocular motility when seen by us. He developed fever one month later,followed by exacerbation of scleritis,blepharoptosis,ocular motility disorder,and impaired right visual acuity. Magnetic resonance imaging（MRI）showed meningeal thickening in the right base of the brain,leading to the diagnosis of hypertrophic pachymeningitis. He showed positive P-ANCA antibody. Systemic corticosteroid with immunosuppressive therapy resulted in improved clinical manifestations. Conclusion：Hypertrophic pachymeningitis may have induced fever and impaired ocular motility in this patient. MRI was useful in its diagnosis.
