Purpose:To report a case of phototoxic retinopathy with subretinal neovascularization. Case and Findings:A 55-year-old male looked at glaring headlight of an automobile for about 20 seconds. He developed hyperemia, pain and impaired visual acuity in both eyes 2 weeks later. His corrected visual acuity was 0.15 right and 0.7 left. Funduscopy showed atrophic retinochoroidal lesion in the right macula and serous retinal detachment with fibrious exudate nasal to the left fovea. Fluorescein angiography showed findings similar to central serous choroidopathy in the left eye. One week later,he started receiving 30 mg prednisolone. Serous detachment in the left eye disappeared 3 weeks after his initial visit. One week later, subretinal hemorrhage appeared at the previous site of fibrinous exudate. Another 7 weeks later, serous retinal detachment appeared again with visual acuity reduced to 0.15. Fluorescein angiography showed subretinal neovascular network. Retinal detachment disappeared 3 months after photocoagulation. Visual acuity recovered to 1.0 one year after onset. Conclusion:This case illustrates that phototoxic retinopathy may develop serous retinal detachment and subretinal neovascularization.
