人眼に6年間挿入されていた摘出アクリル眼内レンズのグリスニング評価

Evaluation of glistening in an acrylic intraocular lens removed six years after insertion

目的：挿入してから6年後に摘出された眼内レンズ（IOL）でのグリスニング発生状況の実験的検索。症例と方法：86歳男性の両眼に超音波乳化吸引術とアクリルIOLの挿入が行われた。その6年後に右眼IOLが下方偏位したためIOLを摘出した。摘出IOLには1mm²あたり100個のグリスニングがあった。生理的食塩水に摘出IOLと同種の未使用IOLを入れ，食塩水の温度を33°から27°の範囲で変化させ，グリスニングの発生状況を観察した。結果：摘出IOLには温度変化の開始直後からグリスニングが生じ，温度が低下するにつれその量が増加した。未使用IOLにはグリスニングの発生がなかった。結論：眼内に挿入されたアクリルIOLは，長期にわたるグリスニングの発現と消失を繰り返していると構造が変わり，グリスニングが発生しやすくなる可能性がある。

Purpose：Experimental production of glistening particles in acrylic intraocular lens（IOL）removed 6 years after insertion. Case and Methods：A 86-year-old male underwent bilateral cataract surgery with insertion of acrylic IOL. The IOL had to be removed 6 years later due to decentration. The removed IOL showed 100glistening particles per mm². We placed the explanted and a new acrylic IOL in saline solution. We then observed the two IOLs by shifting the temperature of the solution from 33 to 27 degrees. Finding：The explanted IOL showed glistening immediately after shifting the temperature. The new IOL showed no glistening throughout. Conclusion：Implanted acrylic IOL may show repeated appearance and disappearance of glistening as long as it stays in the eye. After a while,the IOL may undergo structural changes that facilitate appearance of glistening particles.
