目的：サイトメガロウイルス網膜炎を発症し，硝子体手術を施行した再生不良性貧血の症例の報告。症例：51歳女性が7年前に再生不良性貧血と診断された。輸血と副腎皮質ステロイド剤などで左眼に結膜下出血，両眼に眼底出血が生じた。その後，両眼に硝子体混濁が生じ，硝子体出血と網膜剝離が発症した。硝子体手術で網膜は復位した。多量の網膜下出血，網膜血管の白線化，網膜壊死が術中所見として観察された。血清と硝子体からサイトメガロウイルスDNAが検出され，サイトメガロウイルス網膜炎と診断した。結論：再生不良性貧血にサイトメガロウイルス網膜炎が続発すると，出血傾向のため網膜下出血が起こる可能性がある。
Purpose：To describe a case of aplastic anemia who developed cytomegalovirus retinitis and was successfully treated by vitreous surgery. Case：A 51-year-old female had been diagnosed with aplastic anemia 7 years before. She had been treated by transfusion and systemic corticosteroid. Subconjunctival hemorrhage led to the detection of fundus hemorrhage followed by vitreous hemorrhage and retinal detachment. The retina became reattached after vitreous surgery. Intraoperative findings included massive subretinal hemorrhage,ghost retinal arteries and retinal necrosis. Cytomegalovirus was detected in the serum and the vitreous,leading to the diagnosis of cytomegalovirus retinitis. Conclusion：Subretinal hemorrhage is a liability when cytomegalovirus retinitis develops following aplastic anemia due to hemorrhagic diathesis.
