シリコーンオイル注入眼に発症した前部増殖性硝子体網膜症の4例

Four cases of anterior proliferative vitreoretinopathy following silicone oil tamponade

要約　裂孔原性網膜剝離4例4眼に硝子体手術とシリコーンオイル注入が行われた後に，強い前部増殖性硝子体網膜症が発症した。全例にシリコーンオイル抜去と周辺部の増殖膜切除を行い，3眼で網膜復位を得たが，矯正視力は0.1前後であった。1眼は眼球癆になった。4例に共通した状況として，シリコーンオイル留置が長期間であったことと，初回硝子体手術の際に周辺部硝子体の切除が不十分であったことが，前部増殖性硝子体網膜症の誘因になったと考えられる。このような状況がある眼に対しては，早期にシリコーンオイルを抜去し，周辺部残存硝子体を処理することが望ましい。

Abstract.　Severe anterior proliferative vitreoretinopathy developed in 4 eyes of rhegmatogenous retinal detachment following vitrectomy with silicone oil tamponade. All the 4 eyes were treated by removal of silicone oil and dissection of anterior proliferative membrane. The retina became reattached in 3 eyes resulting in visual acuity of 0.1. One eye became phthisic. All the 4 eyes had two factors in commom：long-term retention of silicone oil and insufficient removal of peripheral vitreous during the initial vitreous surgery. These factors seemed to have induced anterior proliferative vitreoretinopathy. We advocate early removal of silicone oil and extensive removal of peripheral vitreous for eyes with these two factors.
