高度近視黄斑円孔網膜剥離にインドシアニングリーン内境界膜剥離を併用した硝子体手術成績

Outcome of vitrectomy with adjunct use of indocyanine green for macular hole retinal detachment in high myopic eyes

　高度近視に伴う黄斑円孔網膜剥離8例8眼に対して，インドシアニングリーン（indocyaninegreen：ICG）染色を用いた硝子体手術を施行し，その有用性につき検討した。手術では硝子体切除後に，ICGを後極部網膜に塗布したが，その結果，残存硝子体皮質の同定が容易となり，硝子体をより確実に除去することができた。また黄斑円孔周囲の内境界膜を剥離することで，黄斑円孔周囲の網膜接線方向の牽引を軽減できるものと考えられた。術後6か月以上の経過観察で，8眼中7眼は初回手術で復位を得た。術後黄斑円孔は検眼鏡的に閉鎖，もしくは不明瞭となった。ICG染色は，内境界膜剥離を容易にするだけではなく，残存硝子体皮質の確実な除去が可能となり，復位成績を向上させるうえで有用な方法であると考えられた。

We performed vitrectomy on 8 eyes with macular hole retinal detachment and high myopia. Following re-moval of posterior vitreous cortex, we stained the posterior retina with indocyanine green (ICG). This procedure facili-tated identification of remnant vitreous cortex and its removal. Removal of internal limiting membrane (ILM) around the macular hole was performed to decrease the tangential traction to the macular hole. After follow-up for 6 months after surgery, the retina became reattached in 7 eyes. The macular hole became closed or ophthalmoscopically indis-tinct in these eyes. Adjunct use of ICG contributed to a better retinal reattachment by facilitating identification of ILM and residual viteous cortex during surgery.
