Purpose:To evaluate factors involved in reversibility and outcome of diabetic retinopathy after exacerbation induced by strict blood sugar control. Cases:The study involved 20 cases that showed decrease of HbAlc level by 1.0% per month followed by exacerbation of retinopathy. The series comprised 11 males and 9 females. Their ages ranged from 27 to 72 years,average 50 years. There were 2 cases of insulin-dependent and 18 cases of noninsulin-dependent diabetes mellitus. Results:Exacerbated retinopathy was reversible in 6 cases and irreversible In 14. Both groups showed no significant differences regarding:age,type or duration of diabetes,medication,blood pressure,presence of nephropathy or neuropathy,or mode of decrease in HbA1c level. Reversible cases comprised 100% of cases with no retinopathy or AI retinopathy. Irreversible cases comprised 7% of such cases. The difference was significant p=0.0002). All the eyes that had maculopathy before start of blood sugar control showed exacerbation of retinopathy. Conclusion:Strict control of blood sugar may exacerbate retinopathy. Due attention is necessary to the state of retinopathy and maculopathy before start of treatment.
