硝子体手術に至った後部ぶどう腫を有する増殖糖尿病網膜症の1例

Vitreous surgery for proliferative diabetic retinopathy in an eye with posterior staphyloma

目的：後部ぶどう腫がある高度近視眼に増殖糖尿病網膜症が生じ，硝子体手術を必要とした症例の報告。症例：59歳女性が9か月前に糖尿病と診断された。血糖コントロールは良好で，HbA_1c値は6％台にある。前医で両眼に汎網膜光凝固を受けた後，当科を受診した。右眼に－4.5D，左眼に－11Dの近視があり，矯正視力は右0.2，左0.04であった。右眼に増殖前糖尿病網膜症，左眼に後部ぶどう腫，硝子体出血，血管アーケードに沿い増殖膜と牽引性網膜剝離があった。左眼に硝子体手術を行った。後部ぶどう腫周囲に強固な網膜硝子体癒着があった。結論：後部ぶどう腫がある高度近視眼でも増殖糖尿病網膜症が生じる可能性がある。硝子体手術を行う際には，高度近視眼に特有な硝子体変化に留意する必要がある。

Purpose：To report a case of myopia and posterior staphyloma associated with proliferative diabetic retinopathy. Case：A 59-year-old woman had been diagnosed with diabetes mellitus 9 months before. She was referred to us after receiving panretinal photocoagulation in both eyes. She had myopia of －4.5 diopters right and －11 diopters left. She had preproliferative diabetic retinopathy in her right eye. The left eye showed posterior staphyloma,vitreous hemorrhage,and taut vitreoretinal adhesion along the vascular arcades. The left eye was successfully treated by vitreous surgery. Conclusion：Contrary to the accepted view,proliferative diabetic retinopathy may develop in a myopic eye with posterior staphyloma. Due attention is needed to myopia-specific vitreous pathology during vitreous surgery.
