　2歳7カ月，男児。高熱を主訴に近医小児科を受診し，streptococcus pneumoniaeによる化膿性髄膜炎として加療。翌日全身痙攣が出現し，CTにより左前頭頭頂部に硬膜下膿瘍が疑われた。強力な抗生物質による治療を行ったが，患児の活動性は更に低下すると共にCT上硬膜下膿瘍は増大したため，穿頭による排膿ドレナージ術を行った。被膜は黄白色を呈し易出血性で止血困難であり，内容物の大部分は慢性硬膜下血腫様の暗赤色の液体でその中に黄白色の膿を混じていた。通常では，膿瘍内部は粘調な黄白色の膿汁であり，本症例のような所見を呈するものは非常に稀である。発生機序としては，術中の所見より，形成過程にある被膜の豊富で脆弱な新生血管から膿瘍腔に出血をきたしたものと考えられた。

A rare case of subdural empyema accompanied by hemorrhage in a 2-year-old boy is reported.

The patient was hospitalized on May 31, 1995 because of high fever. Lumbar puncture was perfor-med, a diagnosis of suppurative meningitis was made, and streptcoccus pneumoniae was cultured from the CSF. The next day, the patient experi-enced convulsions, and CT scans revealed left sub-dural empyema. Intensive antibiotic therapy was performed for 16 days from the onset of symptoms, but the patient's fever persisted, and the area of subdural empyema had grown larger on CT scan on June 15th.
