　症例54歳，男性。右片麻痺と構音障害で発症したが，それらの症候が改善した後も両側四肢の振動覚，関節位置覚の著明な障害を認めた。MRIでは延髄上部の左錐体から傍正中部にかけて梗塞巣がみられた。体性感覚誘発電位の検査では上肢刺激ではP14の潜時延長がみられ，これは深部感覚障害の程度と相関していた。下肢刺激では両側とも有意な波形は記録できなかった。本例は深部感覚線維が走行する延髄内側毛帯の機能的解剖を理解するうえで参考になる症例と考え報告した。

We report a patient with medial medullary infarc-tion who showed deep sensory impairment as hisprominent neurological manifestation. A 54-year-old man with a history of hypertension was admit-ted to our hospital with numbness of the bilateral upper and lower extremities, followed by dysarthria and right hemiparesis. Physical examination revealed no abnormalities except for high blood pressure. He hiccuped continuously. On neuro-logical examination, he exhibited dysarthria, mild dysphagia and right hemiparesis without facial or lingual paresis.
