　特異なcorticobasal degeneration（CBD）の1剖検例を報告した。症例は，死亡時67歳男性。65歳時右手の不自由感で発症，その後パーキンソニズム，核上性眼球運動障害が出現し，他院で進行性核上性麻痺を疑診，発症9カ月後に痴呆，1年9カ月後に失外套状態となり，全経過2年1ヵ月で死亡。脳重1370g。肉眼的に左下前頭回後部に強調される両側前頭・頭頂葉の萎縮と著明な黒質の退色。組織学的に大脳皮質で2-3層の海綿状態，神経細胞脱落，ballooned neuron多発，線条体・淡蒼球・視床・黒質の神経細胞脱落とグリオーシス，Gallyas-Braak法で広範なargyrophilic threadの出現を認めた。本症例は病初期に核上性眼球運動障害が目立ち，また本疾患としては亜急性の経過を示した特異なCBDである。また，臨床的には明らかな失語を認めなかったが，病理学的に左下前頭回後部の萎縮が目立ちいわゆる緩徐進行性失語との病理学的関連でも興味ある症例である。

We report an unusual autopsy case of cortico-basal degeneration.

The patient was male who was 67 years old at the time of his death. He developed clumsiness of his right hand at age 65. Neurological examination at age 66 revealed supranuclear ophthalmoplegia of upward gaze and parkinsonism. Progressive sup-ranuclear palsy was suspected. The patient's clini-cal course was rapidly progressive. Dementia deve-loped nine months after the onset of the disease, and the patient manifested apallic syndrome at 21 months and died of pneumonia at about 25 months.
