　5-Fluorouracilのmasked compoundであるtegafurによって脳症をひき起こしたと考えられる症例を紹介し，考察を加える。症例は67歳の男性であり，癌性腹膜炎発症後4.5カ月にわたるtegafur投与の後に突然，精神状態の変調をきたした。意識障害は脳波所見によっても明らかとなった。また視覚誘発電位でのP₁₀₀潜時は両側有意に遅延し，尺骨神経での伝導速度は低下していることが確認された。Tegafur投与中止後，患者は臨床症状の改善を示し，脳波，視覚誘発電位，末梢神経伝導速度などの電気生理学的検査も正常に復した。Tegafur投与者において精神症状の出現を認めた際には，臨床所見と共に電気生理学的検査による副作用症状の早期発見の有益性を強調したい。

A case of reversible encephalopthy induced by tegafur:a masked compound of 5-Fluorouracil, is reported and discussed. A 67-year-old Japanese man suddenly began to show changes in mental status after 4.5 months of administration of tegafur for the peritonitis carcinoma-tosa. Alternation in con-sciousness supported by electroencephalografic evi-dence became apparent.The P₁₀₀ latency in visual evoked potential was prolonged in both sides, and the conduction velocity in ulnar nerves in both sides was also slow. The patient made significant clinical recovery upon discontinuing tegafur, and the abnor-mal findings on electroencephalography, visual evo-ked potential and peripheral nerve conduction velocity also returned to normal. When the patients who are taking tegaful show mental symptoms, early diagnosis from both clinical findings and electrophysiological stuides should be performed.
