小児癌治療に伴う脳合併症のmagnetic resonance imaging所見—脳血栓症と白質脳症について

MRI Abnormalities of the Brain in Neurologic Complications following Treatment of Cancer in Children

　小児悪性腫瘍に対する治療中に生じた脳合併症のMRI所見について，そのリスクファクターとの関係から検討した。対象は当施設において過去3年間に治療を受けた脳腫瘍以外の小児悪性腫瘍の50例のうち，脳転移を否定し得たもので，治療中に脳症状を呈した6例を対象とした。発症急性期から経時的に脳MRIを行い，その所見とリスクファクターとの関係を検討した。血液凝固能異常を認めた4例では，病変は皮質と皮質下白質にみられ，脳回表面の造影剤増強効果のみられる例もあり，脳血栓症，特に静脈血栓症を思わせ，その所見は経過とともに軽減した。抗癌剤の髄注などの後に発症した2例では，病変は半卵円中心や脳室周囲白質にみられ，造影剤増強効果はみられず，白質脳症を示唆しており，それらの所見は経過とともに明瞭となった。MRIは，その病変の部位や経過から，それが凝固能異常に基づく病変か自質脳症であるかの鑑別に有用と考えられた。

During a 3-year period, 6 of 50 children with systemic malignacies developed neurologic compli-cations such as hemiparesis, convulsions and loss of consciousness. The children consisted of 1 boy and 5 girls, from 3 to 12 years old, 3 with acute lympho-blastic leukemia and 3 with malignant lymphoma. Four patients recieved induction treatment that included intravenous administration of L-aspara-ginase and/or intrathecal administration of metho-trexate. One patient received induction treatment and consolidation treatment that included intra-venous administration of L-asparaginase.
