喘息をもつ学童を対象とした自記式QOL調査票の改良—改良版（JSCA-QOL Ver.2）の信頼性・妥当性の検討

抄録

　アレルギー外来などで喘息をもつ学童・生徒のQOL傾向をより簡便に測定できる調査票作成を目的として、過去に作成したJSCA-QOL Ver.1（40項目）の調査結果を元に、質問項目数と内容を改良し、5領域30項目と総括スケールで構成されたJSCA-QOL Ver.2を作成した。Ver.2の調査票を用い、関東、東海、四国地方にある20施設の病院・医院でVer.1と同じ小学4年〜高校3年生に配布し、郵送法による回収をおこなった。研究協力への同意は、回収のあった場合に承諾が得られたと判断した。316名の有効回答からVarimax回転による因子分析をおこない、項目の検討を繰りかえした結果、Ver.1と同様の「発作誘発」「日常生活の変化」「家族のサポート」「生活の充実感」「活動への参加制限」の5因子が得られ、高い構成概念妥当性を示した。質問項目を25項目まで絞り込んだ結果、調査票のCronbach's αは0.87と高く、5領域でも0.7以上のαが得られ、高い内的整合性が示された。総括スケールは、「生活の充実感」と強い相関があり、「家族のサポート」とは弱い相関しかなかった。予測された領域に分類されなかった項目があるため、内容妥当性を高めるために質問内容のさらなる検討が必要である。

Abstract

　This paper examined the reliability and validity of JSCA-QOL Ver.2(Japanese school aged children with asthma version 2)questionnaire that revised the JSCA-QOL Ver.1 questionnaire, was revised. The questionnaire has 30 items and a summary scale with a five-point Likert Scale format over five domains: 'factors of asthma attack(domain A)', 'change of daily living(domain B)', 'family support(domain C)', 'satisfaction of daily life(domain D)' and 'limitation on activity(domain E)'. Three hundred sixteen students with asthma aged 10-18 were investigated in 20 hospitals in the Kanto district, Tokai and Shikoku areas. Factor analysis with Varimax rotation, excluding 5 questions that showed a low factor loading(less than 0.4), indicated the same 5 factors, consisting of 25 items, as 5 domains. Cronbach's α for each domain and factor indicated a certain level of internal consistency. There was positive correlation(r_s=0.6) in the summary scale score and total score. The summary scale score had strong positive correlation with the domain of 'satisfaction of daily life'. However, an item(domain D) was classified in factor with a different domain(domain A); therefore, it is needed that we improve the JSCA-QOL questionnaire furthermore by examining the content validity of all items.
