気管支喘息をもつ学童のQOL調査票Ver.1の作成

Development of a Tool to Measure Quality of Life of Japanese School Aged Children with Asthma Ver.1

要旨

　本研究は，10歳以上の学童を対象に作成した4領域40項目で構成された自記式調査票「気管支喘息をもつ学童のQOL調査票Ver.1（JSCA-QOL Ver.1）」の信頼性，妥当性および因子構造を確認し，質問項目の精選，QOLの構成概念について検討することを目的とした．愛知県・岐阜県下11病院小児科外来に通院中の気管支喘息の学童（10歳～18歳）で，本人と保護者から同意が得られた159名を対象に調査し，有効回答総数は142（89.3％）であった．

その結果は以下の通りであった．

1．40項目の調査票全体，「身体」「社会」「感情」の3カテゴリーにおいてはCronbach's α0.6以上を示したが，「家族」は0.44で低く十分な内的整合性は得られなかった．

2．反復再現性（有効回答98名）は，全カテゴリーの級内相関係数が0.6以上と高く，安定性が確認できた．

3．40項目の因子分析では累積寄与率が低く，因子負荷量0.4未満の15項目を除いて25項目で再度因子分析（主因子法，バリマックス回転）を行なった．その結果5因子が抽出され，累積寄与率は48.6％であった．喘息学童のQOLの構成概念として「日常生活の変化」「生活充実感」「発作誘発」「活動制限」「家族のサポート」の5因子が確認された．今後は「家族サポート」項目，総括スケールの追加等の修正について再検討し，より精度の高いQOL調査票を開発する必要がある．

Abstract

　The purpose of this paper were developed the questionnaire measuring the quality of life of Japanese school aged children with asthma Version 1 (JSCA-QOL Ver.1), and choosing the best items of JSCA-QOL Ver.1. The sample for analysis was composed of one hundred forty two children aged from 10 to 18 with asthma. The questionnaire had 40 items with a five-point Likert Scale format over four domains: ‘physical’, ‘social’, ‘familial’ and ‘emotional’. In examining the reliability of the subscales, the results showed internal consistency except for the ‘familial’ domain. Test-retest reliability was almost satisfied for all domains. Factor analysis, excluding 15 questions that showed a low factor loading (less than 0.4), indicated a 5 factors grouping. Those five domains were ‘change of daily living’, ‘factors of asthma attack’, ‘satisfaction of daily life’, ‘family support’ and ‘limitation on the activity’. Thus, it is needed that we will improve JSCA-QOL Ver.1 furthermore.
