腰椎変性すべり症に対する後腹膜鏡視下前方固定術

Anterior interbody fusion utilizing endoscope with retroperitoneal approach for lumbar degenerative spondylolisthesis

　われわれは腰椎変性疾患，特に腰椎変性すべり症に対して後腹膜鏡下に前方固定術を施行してきた．右下側臥位にて，左側腹壁に4つのポートを作製し，後腹膜腔を気腹法にて拡大・保持しながら腰椎左側面にアプローチする．つぎに大腰筋を血管テープにて間欠的に圧排し，当該椎間板の切除，ケージの椎間への挿入を施行する．これまでに28例の変性すべり症に本方法を施行しているが，後方法に比べて侵襲も小さく手技への習熟に伴い手術時間の短縮がみられた．大血管の処理が必要でなく，前縦靭帯，後縦靭帯，椎間関節など腰椎の安定性にかかわる構築要素を温存できるため，有用な用法であると考えた．

We have performed lumbar interbody fusion, utilizing endoscope with retroperitoneal approach for lumbar degenerative spondylolisthesis. Having the patient in the right lateral decubitus position, lumbar spine was ap-proached from the left side. After the retroperitoneal space was approached through dissection of the abdominal muscle layer along its muscle fiber, this space was dilated with the balloon. The dilated space was maintained with CO₂ gas and three additional ports were prepared.
