抄録：高齢者の腰椎変性疾患（主に，変性すべり症）に対する前方椎体固定術（以下，ASF）の術後成績を再検討し，本法適応の是非や問題点を明らかにした．
1971年から1988年の過去18年間に，腰椎変性疾患に対してASFを施行した300例中，60歳以上の21例を対象とした．手術時年齢は平均64歳で，対象疾患は，変性すべり症（以下，DS）14例，分離すべり症4例，椎間板症3例であった．DSについては，50歳未満ASF施行群，後方除圧単独群，後方除圧＋固定術群の3群を対象に高齢者ASF施行群の術後成績を比較検討した．
