横行結腸・下行結腸癌に対する腹腔鏡下手術—安全なリンパ節郭清のポイント

Laparoscopic Assisted Colectomy for Cancer of the Transverse and Descending Colon: Is it Possible to Perform Laparoscopically?

　大腸癌に対する腹腔鏡下大腸切除術（LAC）のなかで横行結腸，下行結腸は手技的に最も難度が高い部位である．横行結腸癌は腹腔鏡下に良好な視野で網嚢を開放し，頭側より横行結腸間膜前葉を剥離して膵下縁を露出し，横行結腸間膜を可及的に菲薄化し，ついで尾側よりリンパ節を郭清するアプローチ方法を選択することで，また，下行結腸癌は内側からのアプローチによるリンパ節郭清を先行することにより安全で合理的かつ繊細なリンパ節郭清が可能となった．ただし，この部位は手技的に習熟してから適応すべきで，困難例には小切開からの郭清，開腹移行も考慮し，安全・確実な手術を心がけるべきである．

Lymph node dissection for cancer of the transverse and descending colon can be performed laparoscopically according to the investigation of the experiences and the improvements of technical aspects. However, the sur-geon must be well trained in the laparoscopic techniques to be able to perform this procedure safely. Thus great care must be taken for the safety of the patient, and the surgeon should not hesitate to convert to extracorporal lymph node dessection or open surgery when the needs arises.
