腹腔鏡下大腸切除術におけるLigaSureTMの有用性の評価

Evaluation of Bipolar Vessel Sealing System for Laparoscopic Colectomy

　腹腔鏡下大腸切除術（LAC）がさらに普及するためには技術的な修練は当然であるが，内視鏡機器の進歩，有効利用も不可欠である．この手術の簡略化の一環として，われわれは新しいシーリングシステムであるLigaSure^TMの有用性について検討した．5mmの腹腔鏡下手術用のLigaSureLAP^TMは若干の改善の余地があるが，特に新しく開発されたLigaSure Atlas^TMは確実な止血効果が得られ，また術式の簡略化を進めるうえで有用で，今後，LACが普及していくうえで有効な機器と考えられた．また患者自身の生体組織を利用したシールが行われるため体内に異物を残すことがなく，これによる利点も考えられた．

In order to spread laparoscopic colectomy (LAC) further, training of the surgical technique is of course nec-essary, as well as the advancement and the effective use of endoscopic surgical instrument. We have evaluated the usefulness of the LigaSure^TM, a new vessel sealing system, as a part of our study for simplifying this surgical technique. Although some product improvements on the 5 mm LigaSure^TM Lap were considered necessary, but with the newly developed LigaSure Atlas^TM produces reliable hemostasis and offers usefulness in simplifying the surgical technique.
