　われわれは胃癌に対するリンパ節郭清を伴う腹腔鏡補助下胃切除術で2000年からLigaSure Lap^TMを，2001年からはさらに改良されたLigaSure Atlas^TMを導入して，現在までに55症例（胃全摘術5例，噴門側胃切除術5例，幽門側胃切除術41例，幽門保存胃切除術4例）に用いた．特に，切離機構付きのLigaSure Atlas^TMは，これのみで左胃動静脈をはじめとするすべての血管を安全かつ簡便にクリップレスで切離でき，合併症もなかった．このLigaSure^TMの腹腔鏡補助下胃切除術への応用は手技の簡略化と安全性の向上に役立ち，この手技の普及に貢献するものと思われた．

This paper details our experience of using two LigaSure^TM devices for laparoscopy-assisted gastrectomy with lymph node dissection in cases of gastric cancer. LigaSure Lap^TM (Lap) was introduced to our department in 2000 and the more advanced LigaSure Atlas^TM (Atlas) in 2001. Since then, the devices have been used in 55 cases (5 for laparoscopy -assisted total gastrectomy, 5 for laparoscopy - assisted proximal gastrectomy, 41 for laparoscopy-assisted distal gastrectomy, and 4 for laparoscopy-assisted pylorus-preserving gastrectomy) . In particular, we found the Atlas,which can cut and separate, has an outstanding safety record.
