Endoscopic surgery has been focused on as a minimally invasive and effective treatment for achalasia. The major objectives of surgery are to relieve dysphagia by Heller myotomy and to prevent gastroesophageal reflux by antireflux procedure such as Dor fundoplication. At present, a well-recognized indication for surgery is for the patients who do not respond to the dilatation therapy. In the near future, endoscopic surgery for achalasia will be very promising as a first line treatment instead of dilatation therapy.
