産婦人科における内視鏡下手術の展望

Application and Future Prospect of Endoscopic Surgery in Obstetrics and Gynecology

　1990年代に急速に普及した内視鏡下手術は，21世紀にはほとんどすべての婦人科疾患の手術に適用されると考えられる．すでにわれわれは不妊症，子宮筋腫，子宮内膜症，卵巣嚢腫，子宮外妊娠などの良性疾患では96％を腹腔鏡下手術ないし子宮鏡下手術で実施し，良好な成績を得ている．したがって，21世紀には内視鏡下手術の教育とトレーニングが卒後研修に必須となる．光学系の進歩にも支えられ，腹腔鏡システムの細径化が進み，より低侵襲手術を可能にし，デイサージェリーや外来手術化も実現しよう．悪性腫瘍への応用は内視鏡下手術のレベルが上がり，十分な治療効果が保証されたうえで適用されていくことが期待される．

Endoscopic surgery is commonly used in the treatment for gynecologic disorders such as infertility, uterine myoma, ovarian cyst, ectopic pregnancy and so on, because of the advantages of this procedure, including minimal invasiveness, minimal stress on the body, and reduced medical and social costs. Indeed, 96% of gynecologic benign disorders are treated endoscopically in our department. Education and training for this special technique will be come more and more important in the gynecologic field.
