　低侵襲冠状動脈バイパス術におけるハーモニック　スカルペルによる胸腔鏡下内胸動脈剥離術の臨床経験を検討した．51例（男／女＝38／13，平均年齢68.5±16.5歳）に対しハーモニック　スカルペルを用い，CO₂送気下胸腔鏡下内胸動脈剥離術（左側48例，右側3例）を行った．6例（11.8％）は再手術例であった．すべての症例において，胸腔鏡下にハーモニック　スカルペルのみにより内胸動脈を第1肋骨上縁から第5肋間まで剥離し得た．採取時間は左側：47±11分，右側：38±8分であった．内胸動脈本幹の解離をきたした1例を除き，50例（98.0％）において術後造影により開存が確認された．胸腔鏡下内胸動脈剥離術において，ハーモニック　スカルペルは有用な凝固器である．

Thoracoscopic internal mammary artery (IMA) harvest using Harmonic Scalpel was reviewed in 51 patients. Thirty-eight patients were male and thirteen were female. The mean age was 68.5±16.5 years. Six were reoperated cases. Forty-eight left and 3 right IMAs were dissected thoracoscopically using Harmonic Scalpel with a hook-shaped blade under 8-10 mmHg CO₂ insufflation into the hemithorax. In all cases, IMA harvest was completed thoracoscopically with excellent hemostasis using only the Harmonic Scalpel. The mean harvest time was 47±11 min in the left and 38±8 min in the right sides.
