This review discusses, from the clinician's viewpoint, what information can and cannot be obtained by pathological examination in patients with connective tissue diseases and vasculitis, and how the pitfalls of these examinations can be avoided. Skin biopsy and temporal artery biopsy are examples of conditions for discussion. There is no single test that can effectively rule in or rule out the diagnosis of connective tissue diseases and vasculitis. Even the pathological diagnosis is not the “final diagnosis”, which should be interpreted in the clinical context. A specimen from an “inflammatory site” must NOT be immersed in formalin. Physicians should obtain adequate specimens to get an accurate pathological diagnosis, and should have a face-to-face discussion with pathologists who can provide readings “between the lines” in reports.
