両側耳下腺内第1鰓溝嚢胞の1症例

Bilateral First Branchial Cleft Cysts within the Parotid Gland

I．緒言

　鰓性嚢胞は通常側頸部に認められる無痛性の腫瘤として出現し徐々に増大する。日常臨床において遭遇することはそれほど珍しくはないが，耳下腺内にこれら鰓性嚢胞が認められることは稀^1〜4）であり，しかも両側耳下腺内に発症する例はきわめて稀^5〜7）である。

　一方本疾患は発正由来や病理組織学的特徴などに関連して，鰓性嚢胞（branchial cyst）という病名のほかリンパ上皮性嚢胞（lymphoepithelialcyst）^1,3）などの病名で呼ばれることもあり，多少混乱を招いている。そこで著者らは今回われわれが経験した稀な1症例を呈示し，あわせて前述したごとき疾患の名称および定義の現状を踏まえつつ，若干の文献的考察を加えて報告する。

　A cystic lesion within the bilateral parotid glands in a 56-year-old woman is reported. The clinical sign, localization and histopathological findings were compatible of the first branchial cleft cyst within the parotid gland. The embryogenesis, diagnosis, and surgical management of these lesions are discussed.
