第二鰓嚢由来の上咽頭嚢胞症例

A Rare Case of Nasopharyngeal Cyst originated from Second Branchial Pouch:A Case Report

　はじめに

　上咽頭に発生する嚢胞は，以前は一様にTorn-waldt氏病^1）と呼ばれていたが，その病因，病態には不明な点が多く，上咽頭に発生する他の嚢胞と混同されていた。

　近年，上咽頭に発生する嚢胞に対する概念や分類が確立されつつある^2〜4）。

　今回，われわれは鰓嚢由来と考えられた上咽頭嚢胞の極めて稀な1例を経験したので報告するとともに，画像診断の有効性と本邦における鰓性嚢胞のついての文献的考察を加える。

Nasopharyngeal cyst is a relatively uncom-mon disease which is considered to be congenital.

A 64-year-old woman was complaining of right hearing loss. A fiberscopic examination, CT and MRI revealed a cystic lesion in the lateral wall of the nasopharynx. We successfully removed the ante-rior wall of the cyst with remarkable clinical improvement. We considered this cyst be originated from the second branchial pouch.
