両側耳下腺に発生した先天性唾液嚢胞の1例

A Case of Congenital Salivary Cysts from Bilateral Parotid Gland

　はじめに

　唾液瘻は一般に炎症や手術後に生じるもので，耳下腺顎下腺などの唾液腺にみられる。瘻孔が皮面にあれば外瘻，口腔内にあれば内瘻という。腺外の主管が障害されたことによる瘻を管瘻といい，腺内の管系が障害されたことによる瘻を腺瘻という。管外に出た唾液が瘻孔を形成せず皮下に貯留すれば唾液嚢胞となる。

　今回われわれは，若年者で，経過から先天性に発症したものと考えられる両側唾液嚢胞を経験し，手術でこれを治癒させることができた。先天性に唾液嚢胞もしくは唾液瘻が形成されることは稀であり，文献的考察を加えて報告する。

Congenital salivary fistulas or salivary cysts are rare.

We described a case of congenital salivary cyst from bilateral parotid gland. A patient was suffered from a swelling and vesicles on the bilateral cheeks. After sialography, sialo-CT and MRI study, we found cysts at subcutaneous tissue of cheek, but out of the parotid gland in each side. We operated on and confirmed the orifices of fistulas at the surface of both parotid glands.
