両側性耳下腺リンパ上皮性嚢胞の1例

A Case of Bilateral Lymphoepithelial Cyst of the Parotid Gland

　はじめに

　リンパ上皮性嚢胞は，一般に側頸部の無痛性腫瘤として認められ，従来は側頸嚢胞と呼ばれていた。その発生由来については鰓溝の遺残による鰓原性嚢胞説，リンパ節内上皮迷入説などがあるが，迷入説を支持する報告や嚢胞の発生由来を証明できない症例では，病理組織学的特徴からリンパ上皮性嚢胞の名称が用いられている。リンパ上皮性嚢胞は，耳下腺に発生することは比較的稀で，両側性に発生することは極めて稀である。今回われわれは，両側耳下腺に発生したリンパ上皮性嚢胞の症例を経験したので，若干の文献的考察を加えて報告する。

A 63-year-old female complained of bilateral swelling of the paroid gland. CT showed a cystic tumor in the left parotid gland and a solid tumor in the right one.

Both tumors were removed surgically but facial nerve was preserved in each side. The origin of the tumors in each side was not identified.

Histopathological diagnosis was lymphoepithelial cyst in which the wall was lined by stratified squamous epithelium and accompanied by abundant lymphoid tissue.

In this case, the tumor was removed with the surrounding normal parotid tissue because of dif-ficulty of preoperative differentiation from other cystic malignant tumors.
