 Clinical and pathological features of malignant histiocytosis, primarily affected the nose, are presented.

 The disease has been described under a wide range of titles : progressive lethal granulomatous ulceration of the nose (Stewart), reticulum cell sarcoma (Walton), and malignant midline reticulosis (Kassel) etc.

 The confusion is based on the pathological features of histiocytosis which shows polymorphic infiltrates with scattered tumor cells and necrosis. The cells are not lymphocytic, but histiocytic because they have phagocytic activity and specific esterase activity, and usually show uncohesiveness.

 Our observations on non-healing granuloma of the nose in past 15 years disclosed that most of the cases consisted of two pathologically different diseases: Wegener's granulomatosis and malignant histiocytosis.
