精索線維性偽腫瘍の1例

Inflammatory Pseudotumor of the Spermatic Cord:A Case Report

　45歳，男性。4年前より徐々に増大する左陰嚢内の無痛性，石様硬の腫瘤を主訴に当科を受診した。超音波検査にて，左精巣を圧排する直径7cm大の腫瘤を認めた。精巣との境界は明瞭であった。明らかな炎症所見やその既往はなかった。左精索腫瘍と診断し，手術を施行した。腫瘤は精索と強固に癒着しており悪性腫瘍も否定できず，腫瘤を含めた左高位精巣摘除術を施行した。病理診断は，精索より発生したと考えられる線維性偽腫瘍であった。

A case of inflammatory pseudotumor of the spermatic cord in a 45-year-old man was described. The patient presented with more than 4-year history of the left intrascrotal stony hard mass which had expanded gradually without pain. There was no evidence of specific history. Ultrasonogram demonstrated the heterogeneous tumor on the left sper-matic cord which measured 7cm in diameter and protrud-ing into the left epididymis and testis but the boundary was sharply retaind. The tumor adhered toughly to the spermatic cord and left high orchiectomy was performed with sus-pected malignancy.
