　81歳男性，示指頭大の無痛性左陰嚢内腫瘤を摘出したところ，精索から出た良性線維性組織球腫であった。2ヵ月後同側鼠径部精索に手拳大の腫瘤が再発し，精索にも硬結を生じたため除睾とともに腫瘤を摘出したところ悪性線維性組織球腫であった。精索の硬結の組織像は前回と同様良性であった。良性と悪性の線維性組織球腫の共存は偶然というよりも，悪性線維性組織球腫の組織の多様性に基づくものと思われた。精索に発生した悪性線維性組織球腫としては本邦6例目である。

An 81-year-old man underwent an excision of a painless small mass in the left spermatic cord. Histological diagnosis was benign fibrous histiocytoma. Two months later, a rapidly enlarging mass as large as a fist developed in the left inguinal canal. The intrascrotal spermatic cord was indurated. The mass and left testis were removed. The mass had a typical histology of malignant fibrous histiocytoma. The induration of the spermatic cord consisted of benign fibrous histiocytoma. The coexistence of benign and malignant forms did not seem to be incidental, but due to pleomorphism of malignant fibrous histiocytoma.
