後腹膜腔鏡下腎摘除術を施行した無機能巨大水腎症

Retroperitoneoscopic Nephrectomy for Nonfunctioning Giant Hydronephrosis:A Case Report

　67歳，男性。レノグラムにて左腎機能の廃絶を認めた。無機能となった左巨大水腎症と診断し，後腹膜腔鏡下腎摘除術を施行した。途中，腎孟内容液を穿刺吸引したが吸引しきれず，腎摘出時に約7cmの皮切を加えた。手術時間415分，出血量100ml。摘出標本は19×11×7cm,370g，腎孟内容液は褐色泥状，2lであった。術後1日目に食事・歩行を開始し，8日目に退院した。術後9日目より復職し，降圧薬も減量した。

A 67-years-old man with a left nonfunctioning gianthydronephrosis underwent retroperitoneoscopic nephre-ctomy successfully. The resected kidney was retrievedfrom the retroperitoneal cavity through the 7cm skin inci-sion.The opeating time was 415 minutes and estimatedblood loss was 100 ml.The size of the resected kidney was190×110×70mm.Pathological examination revealedsevere atrophy and chronic pyelonephritis of the left kidney without malignancy. Postoperative hosptal stay was 8 days.Retroperitoneoscopic nephrectomy can be perforrned withminimal morbidity and shorter postoperative hospital stay.
