女子外陰部悪性黒色腫の1例

A CASE OF MALIGNANT MELANOMA OF THE VULVA:REPORT OF A CASE AND REVIEW OF THE LITERATURE

　60歳，女性。性交時痛を主訴にて近医受診。外陰部の黒色変化を指摘され当院受診。外陰部は尿道口を中心に黒色に変化を来しており8時の方向に腫瘤を1つ認めた。膀胱鏡を施行したところ膀胱から尿道口にかけて異常を認めず。表在リンパ節は触知しなかった。尿道原発の悪性黒色腫として，尿道，子宮全摘出術，腟部分切除術，両側鼠径部リンパ節郭清術および膀胱皮膚瘻造設，有茎皮弁移植術を施行した。術後DTIC，塩酸ニムスチン，ビンクリスチンのDAV療法施行，術後11か月後再発転移を認めず経過観察中である。病理組織学的には異型性を示す紡錘形の上皮細胞が浸潤性に増生し，lentigo様を呈する外陰部原発の悪性黒色腫であった。

A 60-year-old female visited our hospital complaining a pain on sexual intercourse. A block tumor at the 8.0 clock position from the urethral meatus was identified. Urethrectomy and reconstraction of urethra with skin flap, hysterectomy, partial vaginectomy and lymphadenectomy of bilateral inguinal lymph node was performed under the diagnosis of malignant melanoma in April 28 1994. Combination chemotherapy with dacarb-azine, ACNU, vincristine was performed. There is no evidence of recurrence or metastasis untill May 1995.
