超音波検査にて偶然発見した精巣ライディッヒ細胞腫

AN INCIDENTAL TESTICULAR LEYDIG CELL TUMOR WITH MALE INFERTILITY DETECTED BY SONOGRAPHY

　症例は38歳，男性。3年4か月の不妊を主訴に来院。触診にて精巣に萎縮を認める他は正常であったが，超音波検査にて右精巣内に低エコー性腫瘤を認め，カラードプラ法では病変に一致した血流の増加を認めたため，右精巣腫瘍の診断にて高位精巣摘除術を施行。病理組織診断はライディッヒ細胞腫であった。本症例は文献上，触知できず超音波検査にて偶然発見したライディッヒ細胞腫としては本邦1例目にあたる。

A 38-year-old male presented with male infertility for 40 months. He was found to have no abnormality except for testicular atrophy by physical examination. Sonography showed a low echoic mass in his right testis, and color Doppler imaging showed an increased blood flow within the mass. Accordingly we performed high orchidectomy under the diagnosis of right testicular tumor. The patho-logical diagnosis was Leydig cell tumor. This is the first case of incidental and nonpalpable Leydig cell tumor detected by sonography in the Japanese literature.
