膀胱自然破裂による膀胱内回腸脱出

A CASE OF ILEAL DISLOCATION INTO SPONTANEOUSLY RUPTURED URINARY BLADDER DUE TO RADIATION CYSTISIS

　67歳，女性。約20年前に子宮癌の手術と放射線治療を受けた。1990年3月より1年に1回の割合で腸閉塞，腹水貯留，急性腎不全を繰り返し内科的治療にて改善していた。1994年10月19日急性汎発性腹膜炎を発症，穿孔性腹膜炎と診断し，手術を行った。穿孔臓器は膀胱であり腹腔内への尿漏出がみられた。膀胱の他の部位では回腸が膀胱内に脱出して狭窄を起こし腸閉塞の原因となっていた。放射線性膀胱炎に伴う膀胱破裂と膀胱内への回腸脱出による腸閉塞と診断，回腸上行結腸切除術と膀胱修復術を行い治癒した。

The patient is a 67-year-old woman, who underwent radical histerectomy and post operative radiotherapy for uterine cancer twenty years ago. From March 1990 she repeated ileus, ascites and acute renal failure once a year which was managed by non surgical treatment. In October 19, 1994 she experienced severe abdominal pain which was diagnosed as acute generalized peritonitis. We could observe urine leakage from a pinhole rupture at urinary bladder dome and a part of ileum dislocated into the bladder. She underwent the ileo-cecal-colectomy, and closure of urinary bladder.
