自然破裂を来した結石合併尿管瘤

A Case of Spontaneously Ruptured Ureterocele with Lithiasis

　48歳，男性。排便時の怒責後からの肉眼的血尿，残尿感，排尿痛を主訴に受診した。X線検査，超音波検査にて17×14mmの膀胱結石と両側尿管瘤を認めた。膀胱鏡では膀胱結石とともに，浮腫状に肥厚し大きく破裂した右側尿管瘤とこの部よりの出血を認めた。以上から結石を合併した右側尿管瘤が自然破裂し，膀胱内へ結石が移行したものと診断した。経尿道的膀胱結石破砕術のみ施行したが，術後に膀胱尿管逆流現象は認めなかった。

A 48-year-old man who felt dull pain in the right lower back about 6 months previously consulted us with the chief complains of macroscopic hematuria after straining at defecation, residual urine sensation and mild stranguria. X-ray study and ultrasound examination revealed a bladder stone measuring 17×14 mm and bilateral ureteroceles. Cystoscopy revealed a stone and a wide rupture of the right edematous ureterocele and bleeding from the ruptured ureterocele. From the above findings, we diagnosed that spontaneous rupture of the right ureterocele containing a stone yielded the bladder stone.
