尿路疾患に合併した化膿性腰部脊椎炎の3例

THREE CASES OF SUPPURATIVE LUMBAR SPONDYLITIS COMPLICATED WITH URINARY TRACT DISEASE

　急性腎盂腎炎，膀胱全摘術・Kock回腸膀胱造設術後およびTUR-P後に合併した3例の化膿性腰部脊椎炎を経験した．いずれも高熱と腰痛を認め，保存的に治療したが，腰痛は半年以上継続し非常に難治性であった．骨盤腔・後腹膜腔の炎症の後に激烈な腰痛が継続するときにはこのような腰椎の化膿性炎症を考慮にいれておくことが必要と思われた．

Three cases of suppurative lumbar spondylitis with high fever and severe lumbar pain were reported,which ocurred following urologic diseases:pyelonephritis in case 1, total cystectomy and construction ofKock ileal bladder in case 2 and TUR-P and iliopsoas muscle abscess in case 3. All cases were conservativelytreated with antibiotics, analgesics and/or epidural block and corset. It took 9-12 months until the completecure of their clinical signs. These suppurative lumbar spondylitis have to be in mind if severe lumbar painscontinue after the inflammation of retoperitoneal space or pelvic cavity.
