骨盤脂肪腫症の1例

A CASE OF PELVIC LIPOMATOSIS

　会陰部の結核性膿瘍を契機に発見された骨盤脂肪腫症の1例を報告した。症例は49歳，男性，会陰部の腫脹と疼痛を主訴として1986年3月31日入院。17歳時，恥骨部骨結核で治療。腫脹は手術および菌培養で結核性膿瘍と診断できたが，IVP，尿道造影で膀胱底の挙上，下部尿管の中枢側偏位，膀胱の涙滴様変形を認め，CTスキャンで骨盤脂肪腫症の合併が発見された。骨盤脂肪腫症については無症状のため治療は行わず経過観察中である。本症例は本邦第3例目と思われる。

A 49-year-old man was admitted to our clinic on 31, March, 1986, with complaint of perineal purulent swelling and pain. When he was 17 years old, he was suffered from tuberculous ostitis pubis and received curettage and chemotherapy. Urography showed medial deviation of bilateral distal ureters, "tear-shaped" urinary bladder and elevation of the bladder base but no evidence of upper urinary tract dilatation nor tuberculous lesions were confirmed. Pelvic CT-scan revealed that the urinary bladder and the rectosigmoid were surrounded by an abnormal amount of fatty deposit.
