嚢胞内に発生した腎細胞癌

A CASE OF RENAL CELL CARCINOMA ARISING FROM THE RENAL CYST WALL

　同一腎に腎嚢胞と腎腫瘍が合併することは稀であり，診断の困難なことが多い。われわれは術前に診断のついた53歳，男性の1例を経験した。超音波検査，CT，嚢胞内造影，血管撮影で嚢胞内に小さい腫瘍を疑った。嚢胞液は茶褐色で混濁していた。右腎摘除術を行った。嚢胞内にポリポイド様に突出した赤褐色の充実性腫瘍を認めた。組織学的には嚢胞壁内面から発生した腎細胞癌であった。Gibson分類3型と考えられた。

The incidence of cyst and tumor occurring in the same kidney is rare and its accurate diagnosis is difficult. A case of renal cell carcinoma arising from the renal cyst wall in a 53-year-old male is reported. He was diagnosed preoperatively by ultrasonography, computed tomography, selective renal angiography and histochemical examination of the aspirated fluid of the cyst. Right nephrectomy was performed. A solid tumor arised from the cyst wall. Histologically, the tumor was renal cell carcinoma. Gibson outlined 4 types of cysts and tumors arising from the same kidney, and our case belonged to type 3.
