傍腎盂嚢胞による急性腎不全

A CASE OF ACUTE RENAL FAILURE DUE TO PARAPELVIC CYST IN A FUNCTIONALLY SOLITARY KIDNEY

　60歳，男性．無尿にて来院．血液生化学にて尿素窒素，クレアチニンの著明上昇，およびエコーにて両側水腎症を認めたため入院．ただちに経皮約に右腎瘻を造設．CT，逆行性腎盂造影にて右傍腎盂嚢胞による尿管閉塞および左無機能腎と診断し，経皮的嚢胞穿刺およびエタノール注入療法を施行した．尿管閉塞は解除され，術後経過は順調で，6ヵ月経過後も再発を認めなかった．機能的単腎症に傍腎盂嚢胞を併発し急性腎不全に至った報告例は本邦1例目である．

A 60-year-old man visited our hospital with the chief complaint of anuria. The diagnosis of acute renal failure was established by remarkably elevated BUN and serum creatinine. Bilateral hydronephrosis wasfound on the ultrasonogram. Right percutaneous nephrostomy was performed. It was proved by CT scan that a functionally solitary kidney was obstracted by a right parapelvic cyst. Percutaneous puncture and ethanol instillation to the cyst was done. Six months after the procedure, the patient was doing well, and recurrence of parapelvic cyst was not found.
